FAQ:
What forms do I need to fill out for camp?

Overnight and Day Camps:

There are 3 forms required for each camper:

1. Pick-Up Authorization (complete during registration)
2. Health History form (complete during registration)
3. Physical Examination form (in the download/uploads section of your account)
4. Immunization Records (in the download/uploads section of your account)

Please upload your physical examination form and immunization records to Your Account

Eagle Lake On Location:

There is 2 form required for each camper:

1. Health History form (complete during registration)
2. Pick-Up Authorization (complete during registration)

If someone other than the person who registered the camper is picking up your camper during the week, please be sure to include their contact information under ‘Authorized Pick-up’ under your Account Dashboard.
